Meridian Health Check is a free referral service based in Miami, Florida. We connect adults in Florida with independent licensed healthcare providers. We are not a medical provider.
What we collect
- What you enter in our forms: name, phone number, email, ZIP code, and your answers to the symptom questions.
- Basic usage data collected by Google Analytics and Google Ads through cookies, such as pages visited and the ad that brought you here.
How we use it
- To pass your request to one licensed provider partner so they can contact you about scheduling labs and a consultation.
- To contact you by call, text or email about your request, if you gave consent.
- To measure and improve our website and ads.
We do not sell your information to data brokers. We share your request with one provider partner at a time. Provider partners may pay us a referral fee.
Where it is stored
Form submissions are stored in Google Forms and Google Sheets under our account. Access is limited to Meridian Health Check.
Your choices
- Reply STOP to any text message to stop texts.
- Email us to ask for a copy of your data or to have it deleted.
- You can block or clear cookies in your browser settings.
